Benefit Packages
Pullman Regional Hospital offers a comprehensive benefits package that includes Medical Insurance, Vision Insurance Dental Insurance Life and AD&D Insurance and Long Term Disability Insurance.
Medical Insurance
Annual individual deductible is $200, and $600 per family. After the deductible is met the insurance pays 80% with the employee co-payment at 20%. There is a 1 million dollar lifetime maximum on the plan. Coverage is the basic medical plan coverage with plan enhancements to include: diabetes education, massage therapy and a wellness benefit that covers annual physicals up to $100 with no deductible or co-pay. Pullman Regional Hospital pays 100% of the insurance premium for the employee. Employees pay 100% for dependent coverage through payroll deductions.
Vision Insurance
Annual deductible is $20. Eye exams are paid for once every 12 months. Lens are paid for once every 12 months and frames are paid for once every 24 months. Pullman Regional Hospital pays 100% of the insurance premium for the employee. Employees pay 100% for dependent coverage through payroll deductions.
Dental Insurance
Annual deductible is $25 for an individual and $75 per family. However the deductible is waived for preventive services. The insurance company pays 100% for preventive services, 80% for basic services, and 50% for major services. Pullman Regional Hospital pays 100% of the insurance premium for the employee. Employees pay 100% for dependent coverage through payroll deductions.
Life and AD&D Insurance
Both plans provide Employee Only coverage. Coverage for both plans is 1x annual salary up to a maximum of $50,000.
Long Term Disability Insurance
This plan provides coverage to employees who are totally or partially disabled and are unable to return to work. The coverage begins 180 days after the disability and covers 66 2/3% of earnings. |
